Most of this involves dumb ass shorthairs like me trying to mosh as hard as the long hairs. Which is why I just stick to banging heads with people and dropping the shoulder. Pain is gain, if you go to a show and your neck doesnt hurt, your voice isn't sore, and you haven't lost anything(shoes,hats,etc) than you haven't been to a show.

i used to headbang for whole gigs when i played in a rock/metal covers band. it's gotten to the point now that if i headbang for just a few mins, it hurts a bit, and then i can barely move my neck in the morning.
now if i get the urge to headbang, i do the kinda upper-body-bang thing, where there's less impact on the neck (and looks more badass). altho usually i stick to the whole fist/finger in the air and shouting the lyrics, and jumping about. and ball-punching/stealing the wallets of any crowd surfer that feels the need to kick me in the head more than could be deemed accidental...

It just takes training. Once you've passed a certain limit, you can headbang without any reprecussions. I remember a year or so again I had pretty bad bangovers, but these days I can bang a long time, and... survive? :D
