When you dig a little deeper there could be more behind the lyrics then you'd think. Originally, most of the song was taken from a poem Till Lindemann wrote, in which he was talking about his mother, not his sister. There's a theory that in the song, it is actually his mother, not his sister. Because of his mother trying to protect him from being called a 'whoreson' he is asked to call her 'Schwesterlein'.
The medicine he has to take is also interesting. It might be a sleeping draft or some kind of drug to keep him from making too much noise while she is 'working' next door, so she and her clients do not get disturbed.
He takes his frustrations out on the doll which substitutes for his sister/mother and almost seems to act as some kind of voodoo doll in the final chorus.

"Der Songtext bezieht sich in Teilen auf das Gedicht „Wenn Mutti spät zur Arbeit geht“ von Till Lindemann, das in dem Buch „In stillen Nächten“ auf Seite 24, 25 zu finden ist.
Im Song „PUPPE“ wird aus der Mutter die Schwester, einige Zeilen und Strophen werden direkt aus dem Gedicht übernommen, einige erscheinen abgewandelt. Neu dazuaddiert ist die Puppe als Leitmotiv."
Dazuaddiert is my new favourite word.

First 6 songs are fucking gold. In the last 3 songs is where it looses steam for me (being good songs though)

that's a very tough question...maybe between Puppe and Hallomann but I remember there being a ton of awesome riffs mid album that I don't seem to get to quite as often so that could change
The opening duo of Deutschland/Radio is also immensely satisfying
