It's the fact that she is skinny and attractive, but labeling herself as fat as the issue, not the word fat. Her fanbase is mostly teenage females who are VERY impressionable and them seeing Taylor labeled as fat is not good for their own self image. We are all grown men here, of course it looks different to us.
But again it's really not THAT big of a deal, Twitter always takes it too far

True. On one hand, it’s understandable for her to have said insecurities and struggle with all the ways that impacts her life and other’s expectations and on the other, to the impact of all that you’ve said. Balance in this seems tricky and requiring finesse, and it’s gnarly and exhausting to think about all at once sometimes.
Especially when in her position, where self-expression comes with a level of influence that can go pretty deep.
