Are There Any Bands....
 
....who have had at some point none of the original members who started it originally?

I've always wondered this but never come across any. Also at what point would a band stop being authentic to the name and the members really should change it?

Mikael Akerfeldt has been in Opeth since the beginning when you consider that it wasn't Opeth before he joined.

Napalm Death is the only one I know of.

2 out of 6 from the dead are in fact dead. the other 4 still play. some more than others. but the dead havent played together in a long time anyway

Negative, the Grateful Dead had four members that played every show with them. After Jerry died, the remaining members toured as the Dead (technically a different name) a few times.
