Which wire is positive in a Jack lead? The thick, outer one, or the thinnder one inside the second lot of insulation? and thich corresponds to which part of the plug?

The inner core is usually used for the 'live' signal wire. The outer shielding wire is usually used for the ground.

This inner core should be soldered to the tip connection and the outer shielding wire should be solderd onto the sleeve connection of 1/4" plugs.
